CMS reports separate health-inspection, staffing, and quality-measure ratings alongside an overall rating. These measures apply to nursing homes; they are not a shared rating scale for Maine’s assisted-housing licenses.

Consider travel for regular visits as well as the published measures. Ask the home about the specific care needed, current staffing, what payment arrangements it accepts, and any costs not covered by those arrangements.
